Now taking appointments. Please browse around as there are new offerings to the menu.  Rates and loyalty and referral rewards have also changed.  You can find the intake or physician permission forms in the tab marked as such. You can print, fill out and bring the intake or bodymap with you, or you can fill one out when you get here. If you are under the regular care of a physician, you must bring a signed permission form from your physician. You can find these forms under the tab marked "client forms".
